REQUEST FOR PROPOSALS

GROUP HEALTH PLANS
BID NUMBER RFP 2008 – 015

The City of Lauderhill is accepting proposals from qualified providers to provide Group Health Insurance to full-time employees until 2:45 P.M. EST on Wednesday, July 9, 2008 at the City of Lauderhill, City Clerk’s Office, 3800 Inverrary Boulevard, Suite 301, Lauderhill, Florida 33319 and will be opened at 3.00 P.M. in the Multipurpose Room, Suite 202.

Group Plan includes PPO & HMO coverage as well as Group Medicare Plan. Interested parties may obtain copies of this Request for Proposal by calling the City Clerk’s Office at (954) 730-3011.

Bids received after 2:45 P.M. will not be considered and will be returned to the bidder unopened. Proposers should submit an original and three (3) copies of a written proposal. The original proposal and copies should be submitted in 3-ring binders in a sealed envelope bearing the words: “BID 2008 – 015: “Group Health Plans.”
E-mailed and faxed proposals will not be accepted.

The City Commission of the City of Lauderhill reserves the right to reject any and all bids, to waive any and all informalities or irregularities and to accept or reject all or any part of any bid as they may deem to be in the interest of the citizens of the City of Lauderhill.
